  • member in good standing

* This eligibility criteria is encouraged but not required for nominated medical students and family medicine residents.

Nomination Process

The ACFP Awards and Recognition Committee only accepts completed nominations, which provide details on how the nominee meets the criteria. Additional letters of support from colleagues, patients, and members of the community are also strongly encouraged in your submission.

Alberta and Beyond

Take your nomination to the next level — apply for one of the grants, scholarships, or awards offered by the College of Family Physicians of Canada (CFPC)!

Through its Foundation of Advancing Family Medicine (FAFM), the CFPC’s awards program supports the training and continuing professional development of medical students, family medicine residents, and family physicians.

FAFM Honours and Awards

Award Criteria

This award recognizes outstanding family physicians who best exemplify what being a family doctor is all about: providing exceptional care to their patients, making significant contributions to the health and wellbeing of their community and/or society in general, and dedicating themselves to the education of future generations of family doctors. As part of the adjudication process, the successful recipient will also be nominated as the Reg L. Perkin Family Physician of the Year for Alberta, bestowed by the College of Family Physicians of Canada. Recipients are nominated by their peers.

Award Criteria

  • Nominee is an Active Member, as defined by the CFPC Membership Policy (Non-Member Mainpro+® Participants are not eligible).
  • Nominee is an ACFP member in good standing and has been in practice for a minimum of 10 years.
  • Nominee is NOT a current member of the ACFP Board or the Awards and Recognition Committee.
  • Demonstrates outstanding leadership in the areas of advocacy, community service and/or education.
  • Supports excellence in family medicine, including community medical services, involvement in the College and contribution beyond their medical practice to the community at large.
  • Exemplifies the four principles of family medicine:
    • The family physician is a skilled clinician.
    • Family medicine is a community-based discipline.
    • The family physician is a resource to a defined practice population.
    • The patient-physician relationship is central to the role of the family physician.

The Recognition of Excellence Awards recognize members who, in the past 12-24 months have made an outstanding contribution in one or more of the following areas: patient care, community service, hospital or health care institutions, College activities, teaching, research, and other elements of the academic discipline of family medicine. Examples of outstanding contributions may include, but are not limited to the following:

  • Significant humanitarian work on a global basis.
  • Establishment of a training program.
  • Delivery of an educational program unique to the needs of an at-risk population.
  • Excellence in clinical practice to disadvantaged communities.
  • Leadership in primary care renewal projects, hospitals or program.
  • Development of an innovative research project that contributes to the improvement of day-to-day clinical practice.

As part of the adjudication process, successful recipients will also be nominated for the Awards of Excellence, bestowed by the College of Family Physicians of Canada. Recipients are nominated by their peers or members of the public.

Award Criteria

  • Nominee is an Active Member, as defined by the CFPC Membership Policy (Non-Member Mainpro+® Participants are not eligible).
  • Nominee is an ACFP member in good standing.
  • Nominee has made an outstanding contribution in the last 12-24 months in one or more of the following areas: patient care, community service, hospital or health care institutions, College activities, teaching, research, and other elements of the academic discipline of family medicine.
  • Nominee is NOT a current member of the ACFP Board or the Awards and Recognition Committee.

As a chapter of the College of Family Physicians of Canada, the ACFP supports the vision of a Patient’s Medical Home (PMH), a model of care that puts the needs of patients and their communities at the centre of care. The PMH is where patients can present and discuss their personal and family health concerns and receive the full spectrum of expert care. The PMH team is developed to meet the needs of its own patient community and, depending on those needs, may involve nurses, other specialists, and other health care providers, who work together in one centre or virtually. The PMH supports the relationships between patients and family physicians and other health care providers as they develop and strengthen over time, enabling the best possible health outcomes for each person, practice population, and the community being served. Recipients are nominated by their peers, members of the public, or are self-nominated*.

Award Criteria

  • The family practice or clinic has been in operation for a minimum two years.
  • Demonstrates outstanding leadership in the areas of patient and community care while exhibiting excellence in the application of the principles of the Patient’s Medical Home.

*Clinics, where current members of the ACFP Board and Awards and Recognition Committee practise, are still eligible for the award; however, during the review process the said Committee member will abstain from the voting and review preference.

The Family Medicine Resident Leadership Award supports, promotes, and recognizes residents, emerging leaders and advocates for family medicine. It is awarded to two family medicine residents (one from the University for Alberta and one from the University of Calgary) who exemplify a broad-based understanding and demonstration of family medicine. Recipients are nominated by their peers, members of the public, or are self-nominated.

Award Criteria

  • Nominee is a family medicine resident enrolled at either University of Alberta or the University of Calgary.
  • Nominee is NOT a current member of the ACFP Board or the Awards and Recognition Committee.
  • Demonstrates strong leadership skills in advocacy, community service, and/or education.

The Student Leadership – Rising Star Award supports, promotes, and recognizes students, our future leaders and advocates for family medicine. It is awarded to two medical students (one from the University for Alberta and one from the University of Calgary). Recipients are nominated by their peers or members of the public, or are self-nominated.

Award Criteria

  • Nominee is a student enrolled in the undergraduate medical degree program at either University of Alberta or the University of Calgary.
  • Nominee is NOT a current member of the ACFP Board or the Awards and Recognition Committee.
  • Demonstrates a genuine interest in family medicine which is reflected in his or her experiences and chosen electives.
  • Participates in various family medicine activities during their studies (e.g. participation in a Family Medicine Interest Group, Student Information Session, Shadowing Program or attending the Annual Scientific Assembly.)
  • Demonstrates strong leadership skills in advocacy, community service, and/or education.

The New Professional Award recognizes a family physician, in his or her first five years of practice, who has not only successfully established his/her career, but has demonstrated balance through commitment to family, patients, colleagues and community. Examples may include, but are not limited to the following:

  • Served as a volunteer in the community.
  • Demonstrated proficiency and competency in providing care to patients.
  • Opened a new practice – solo or group.
  • Contributed to a new practice or locum, by sharing on call duties, working with colleagues and other health professionals in a team environment.
  • Participated as a valued member of a professional committee.
  • Provided leadership through involvement in teaching and/ or research.
  • Improved problem solving skills in the clinic and/ or hospital setting.

Recipients are nominated by their peers or members of the public, or are self-nominated.
